The first step in adding security questions to your WordPress login screen is to install and activate the Two Factor Authentication plugin. The free version of this plugin allows you to set up security questions for up to three users with administrator roles. Additional users or users with non-administrator roles will require a premium license.
Configuring Your Security Questions
After activating the plugin, go to the “Multi-factor Authentication » Two Factor” page to configure it. Scroll down to the Security Questions section and click on the “Configure” button at the bottom of the page.
You will find a list of pre-defined questions in dropdown menus. Choose two questions from the list and manually enter your third question. Then, provide the answers to your selected questions. Don’t forget to click the “Save” button to save your settings.
